submit this form directly - it must be submitted by your
child's teacher, along with their registration forms. (Your
child can not participate without signed form)
I grant permission to National Bee, Inc.
and its subsidiaries and affiliates and their respective
employees and agents (all referred to in this Release as
"National Bee") and any photographer, Videographer or other
person hired by National Bee to photographer, videotape
and record the participant in connection with the Numbers
Bee program. I understand and agree that National Bee may
reproduce, broadcast, distribute and publish the photographs,
video and recordings, in addition to the participant's name,
likeness, voice and relevant information, for promotional
and all other legal uses in various media throughout the
world, including without limitation newsletters, brochures,
marketing materials, newspapers, press releases and other
print media, television and computer on-line services. I
understand and agree that neither the participant nor I
will receive any compensation for the use of the photographs,
videos and recordings, and that all photographs, videos
and recordings and all intellectual property rights related
to then will be owned solely by National Bee.
Each participant agrees to abide by tournament
rules and regulations set forth by National Bee. Any decisions,
judgments and /or procedural adjustments made by National
Bee personnel or volunteer proctors before or during the
event are final once the events has concluded.
I represent and promise that I am the parent
or legal guardian of the participant, that I am of legal age,
and that I have the authority to sign this release of behalf
of the participant.
Please return this forms nothing your granting/denying
permission for your child to participate.
